Social Prescribing Service at Selby District AVS

How does it work?

Social Prescribing is a great way to find out about extra services that may support you to improve your health, wellbeing, fitness and independence.

If you think the service may help you or someone you care for you can refer yourself to the service by contacting Community House on 01757 291111. Alternatively you can speak to your GP or member of your practice team and they will be able to request a referral.

The social prescriber is...

Someone to talk to openly and confidently to. Someone who is practical, helpful and who will not judge you. Someone who can help you decide on what you would like to improve on wether it be health, wellbeing or independance. Someone who will help find activities that will suit you and will give support along the way.
